Central Coast Primary Care are seeking enthusiastic and dedicated people to join our team of Home and Community Support Workers in providing supports to our National Disability Insurance Scheme (NDIS) participants.  These supports will allow people to live independently, to access and integrate into their community and to build skills and access appropriate services. You will provide support in their day to day lives by assisting and encouraging participation in daily tasks, support and promote daily and community activity participation.

It is essential that you have the following:

  • Driver’s license and access to a reliable and comprehensively insured motor vehicle
  • A commitment and ability to work with people who have mental health and / or other disabilities
  • Have a Certificate III in Mental Health, Disability, Community Services, Assistant in Nursing.
  • Prepared to work at least 20 hours a week
  • Competent with using smart phone applications
  • National Police Check including working with vulnerable people
  • Working with Children Check in paid employment

It is desirable that you have:

  • First Aid Certificate
  • Experience working with people with a Disability
