Coast Nutrition

Rationale

Coast Nutrition provides a FREE Dietitian service to frail, older people and their carers through a government funded program. This service is provided to clients in their own home. CCPC gratefully acknowledges the funding from Department of Health for this service.

The focus is on those who are malnourished or at risk of malnutrition, hence it may include individuals with a variety of diseases and chronic conditions including pulmonary, renal or cardiovascular disease, gastrointestinal disorders, diabetes, cancer, dementia and those with mental health conditions.

Individuals who are needing general healthy eating or weight reduction advice in the absence of significant other medical issues are not appropriate for this service.

People who are malnourished or at increased risk of malnutrition benefit most from the Coast Nutrition service.

Talk to us about the range of services that  Central Coast Primary Care can provide NDIS participants inclusive of ‘Support Coordination’  which can help with putting all aspects of your NDIS plan in place.
